Lower-limb wound evaluation and care in the vascular surgery patient.

چکیده
This issue of Seminars in Vascular Surgery is devoted to the evaluation and management of lower-limb wounds in the vascular surgery patient. In-depth discussions of common clinical conditions are presented on the topics of venous ulcer, diabetic foot infection, peripheral arterial disease (PAD) with tissue loss, and surgical site infection (SSI). The first article by Scalise et al discusses the role of bacterial biofilms and the wound microenvironment on healing. The treatment of surface bacterial biofilms is critical to the treatment of all lower-limb wounds and for the prevention of SSI. When a chronic lower-limb wound is encountered, its evaluation and management require a multifaceted approach that addresses the complex factors involved in the healing process of cellular proliferation and angiogenesis. These authors detail the biofilm-based strategies that should be considered for healing lower-limb wounds caused by trauma (diabetic foot ulcer), venous insufficiency, and PAD. In their article on the management of venous ulcer, Cooper et al detail the disability of this common vascular condition, including its associated decreased quality of life, reduced mobility, and social isolation. Treatment of this vascular lower-limb wound requires wound debridement, treatment of biofilm infection, compression therapy, and a wound environment conducive for healing. These authors emphasize that collagen and antimicrobial dressings can accelerate ulcer healing. Current vascular surgery practice is to eliminate venous duplex ultrasound documented truncal superficial (great and small saphenous, perforator) vein reflux when ulcer healing has not been achieved within a 3month period of compression therapy and wound care. The evaluation of patients with diabetic foot ulcers is addressed in two articles. The health-related quality of life in these patients is detailed in the article by Sekhar et al, with emphasis on both the physical and mental health aspects. The presence of a foot ulcer in a diabetic patient is a limbthreatening condition and requires the health care team to provide patient-specific education and wound care. In a companion article by Nteleki et al, the contemporary

Comparison of therapeutic results of large saphenous vein resection with conventional methods and standard bridging
Background: The use of the traditional method for saphenous vein harvesting is associated with wound complications and not on-time patient mobilization. This has caused the improvement of minimally invasive vein harvesting techniques, together with general bridging. This study was designed to compare the therapeutic results of...
متن کاملUpper limb deep vein thromboembolism: a case report
Upper limbs deep vein thrombosis is a rare event in patients, although increasing incidence is seen due to higher cancer diagnosis rate and the use of various vascular catheters. A 58-year-old man with a history of anemia was scheduled for radical prostatectomy due to prostate adenocarcinoma. The patient underwent surgery under general anesthesia. After surgery, he was transferred to the intens...

Background & Aims: Vascular trauma is an emergency limb-threatening situation and the third leading cause of mortality and morbidity (amputation) all over the world which can lead to significant complications due to lack of correct diagnosis. The aim of this study is to evaluate the frequency of various orthopedic injuries in traumatic patients requiring vascular surgery interventions. Materia...
متن کاملFalse positive lower limb lymphoscintigraphy due to inadvertent intra-vascular injection of the tracer: Importance of tracer re-injection
We presented a 30 year-old woman presented with symmetrical swelling of the lower limbs since 5 years ago with sparing of the feet. She was highly suspicious of having lipoedema and was referred for lower limb lymphoscintigraphy to rule out possible lymphedema. Lymphoscintigraphy images showed asymmetrical injection sites and unusually high liver and spleen uptake. Inguinal nodes was not visual...
متن کاملEvaluation of the Effects of Adipose Derived Mesenchymal Stem Cells Cultured on Decellularized Amniotic Membrane in Wound Healing of Distal Part of the Limbs in Horse
Background: Equine wounds, particularly those involving the distal portion of the limbs, often undergo prolonged complex healing and may enter a non-healing state with obvious financial and welfare implications. Poor blood supply and lower levels of oxygen to the lower limbs lead to imbalance of growth factors related to wound healing. Decellular amniotic membrane has been widely used in soft t...
